Click on the link below and complete the forms. Once you have completed the forms online, print the forms and return them to Rivera Middle School on your scheduled date. A separate packet must be completed for each child that attends Rivera Middle School.
Current ERUSD students:
Please make sure to download all the necessary paperwork and fill out the forms and return them signed on the date below.
August 13th, 8th grade- 8 am- 11am and 1pm- 3pm in the library
August 14th, 7th grade- 8 am- 11am and 1pm- 3pm in the library (If you have not provided proof of your TDAP please make sure to bring the proof in order to pick up your schedule)
August 17th, 6th grade- 8 am- 11am and 1pm- 3pm in the library
August 18th, Any student who could not make there assigned date- 8 am- 11am and 1pm- 3pm in the library
